Online Waiver Form

If you have not become a Shuttlecats member, please complete the online waiver form when you are:

1. Playing as a guest at one of our sessions
2. Taking part in a tournament or event run by Shuttlecats
3. Whenever you are asked to as part of the registration procedure for any Shuttlecats event.

You only need to do this once, it will cover you for every session you attend as a guest.
